Why not an online service

Real labs, real visits, real accountability.

Online GLP-1 services sell access to a prescription. They do a questionnaire, ship a vial, and disappear until you want a refill. The problem is that semaglutide is a medication, not a supplement. Clinical evidence

What the research actually shows.

Semaglutide for weight management was studied in the STEP trial program (Semaglutide Treatment Effect in People with Obesity). The flagship trial, STEP 1 (Wilding et al., New England Journal of Medicine, 2021), followed 1,961 adults with obesity for 68 weeks. The average weight loss on 2.4 mg weekly was 14.9% of starting body weight, compared with 2.4% for placebo. About one in three participants lost more than 20% of their body weight.

Cardiovascular benefit was confirmed in SELECT (Lincoff et al., NEJM, 2023), which showed a 20% relative reduction in major adverse cardiovascular events among patients with overweight or obesity and preexisting cardiovascular disease. These findings were the basis for Wegovy’s FDA label update for cardiovascular risk reduction.

Most patients notice appetite change within the first two weeks. Visible weight loss usually begins around week four to six and builds as the dose titrates upward. The slow-and-steady pattern is part of the safety profile, which is why we resist compressing the titration even when patients want faster results.

What to expect

Your first 90 days, week by week.

Week 1–2. Starting dose of 0.25 mg weekly. Most patients feel reduced appetite within days. Some mild nausea or fullness is common; it almost always improves by week three. We send tips on meal timing, hydration, and protein targets.

Week 3–4. Dose increases to 0.5 mg. Appetite suppression is noticeably stronger. Many patients start seeing 2–4 lbs of weight loss per week. We schedule a 15-minute virtual check-in to troubleshoot any side effects.

Week 5–8. Dose advances to 1.0 mg if tolerated. This is where most patients hit their groove. You’re eating smaller portions naturally, not thinking about food between meals, and noticing clothing fit differently.

Week 9–12. Most patients settle into 1.0–1.7 mg as a working dose. We repeat a brief lab panel to confirm kidney and thyroid function remain stable. From here, care is monthly and focused on long-term behavior change plus dose management.

Brand-name semaglutide is no longer the thousand-dollar-a-month product it was two years ago. Novo Nordisk now sells Wegovy directly to self-pay patients at a fraction of the old pharmacy counter price, and the whole market has moved with it. We are not going to build a pitch on savings that no longer exist.

So here is the straight version. If the lowest possible price on the molecule is your only variable, buying a brand pen direct is a legitimate option, and we will tell you so at your consult rather than talk you out of it. What we charge for is not a discount on a drug. It is the medicine around the drug: the physical exam, the baseline and follow-up labs, the monthly InBody scan, dose decisions made on your data instead of a shipping calendar, and a physician who answers when something goes wrong at 8pm on a Sunday.

We are also not locked into compounded. If a brand product is the better fit for your dose, your insurance, or your tolerance, we will write it and manage you on it. The care is the same either way.

Program pricing depends on your dose and the length of the program, and covers the medication, supplies, your monthly in-person visit, and your body composition scans. Labs are billed separately at wholesale. We do not bill insurance directly for compounded medications, but we provide superbills for HSA or FSA reimbursement and offer CareCredit for patients financing a full 12-month program up front. How much weight will I lose on semaglutide?

In the STEP 1 trial, patients lost an average of 14.9% of their starting body weight over 68 weeks at the full 2.4 mg dose. Real-world results vary with nutrition, activity, and how long you stay on therapy. Most of our patients land in the 10 to 15% range over the first year.

How quickly will I see results?

Most people notice reduced appetite within the first two weeks. Visible weight loss usually starts by week four to six and continues steadily as your dose titrates upward. The biggest changes happen between months three and nine.

What are the common side effects?

Nausea, reflux, constipation, and early fullness are the most common, and they are almost always dose-related. They tend to improve after the first few weeks. We titrate slowly to minimize them, and we have a detailed toolkit for managing each one.

Are there dangerous side effects to watch for?

Rarely, GLP-1 therapy can trigger pancreatitis, gallbladder disease, or worsen preexisting gastroparesis. We screen for these risks at your initial visit and teach you the symptoms to report. Your labs catch most issues before they become problems.

Is compounded semaglutide the same as Ozempic or Wegovy?

The active ingredient is the same. Ozempic (type 2 diabetes) and Wegovy (weight loss) are Novo Nordisk brand products; ours is prepared by a licensed US compounding pharmacy. Brand self-pay pricing has come down a great deal, so the two are much closer on cost than they used to be. We prescribe both and will recommend whichever fits your dose, tolerance, and budget at the consult.

How much does the program cost?

The monthly program covers the medication, supplies, your in-person follow-up visit with the physician, and your InBody body composition scan. Pricing depends on your dose and program length. Can I stop taking it once I reach my goal?

Many patients taper to a maintenance dose once they hit their target. Stopping abruptly often leads to appetite returning and weight regain within three to six months, so we wean down gradually under supervision.

Can I use semaglutide with other medications?

In most cases yes, but we review your full medication list at the consult. There are specific interactions (oral contraceptives, certain diabetes meds, thyroid replacement) that we account for. If you’re on insulin or sulfonylureas, dosing needs a careful adjustment.

What if semaglutide stops working for me?

A plateau around month six is not unusual. We troubleshoot nutrition, activity, and dose first. If you’ve plateaued despite optimization, we may switch you to compounded tirzepatide, which works on a second receptor pathway.

Wegovy is much cheaper than it used to be. Why use a clinic at all?

Fair question, and the price drop is real. What a pharmacy or a website sells you is the drug. What we sell is the drug plus the medicine around it: a physical exam, labs that get repeated, a monthly body composition scan so we can prove you are losing fat rather than muscle, dose decisions made on your data, and a physician who picks up the phone when something goes wrong. If you would rather have the vial and manage the rest yourself, that is a reasonable choice and we will say so. We just will not pretend the two are the same thing.

Do you prescribe brand-name Wegovy or Ozempic as well?

Yes. We are not locked into compounded medication. If the brand product is the better fit for your dose, your coverage, or your tolerance, we will write it and manage you on it. The exam, labs, scans, and follow-up are identical either way.

What is an InBody scan, and why do it every month?

InBody is a body composition analyzer. You stand on it for about a minute and it separates your weight into fat mass, skeletal muscle mass, and body water. It matters on GLP-1 therapy because a meaningful share of rapid weight loss can come from muscle if nobody is watching. The monthly scan is what tells us whether to advance your dose, hold it, or fix your protein and training first. A scale at home cannot tell you any of that.

Do I really have to come in every month?

Yes, and it is the part of the program that does the most work. The monthly visit is where the exam happens, where the scan gets repeated, and where your dose actually gets decided. Patients who skip in-person follow-up are the ones who plateau on the wrong dose, lose muscle without knowing it, or sit on a side effect that should have been addressed weeks earlier. Visits run about 20 minutes and we keep to schedule.
