Almost everyone has stood in front of a mirror, tilted their head down, and wondered where that soft fullness under the chin came from. A double chin can show up on people who are lean and fit, and it can hang around even after weight loss. That’s the part most people don’t expect. It isn’t always about the number on the scale.

The area under the chin, which providers call the submental region, is influenced by a handful of different things, and the right treatment depends entirely on which of those things is driving it for you. This guide walks through the real causes and gives you an honest look at what can and can’t be done, including where Olympia’s menu fits and where it doesn’t.
What Actually Causes a Double Chin
“Double chin” is the everyday phrase. The clinical term is submental fullness, and it describes extra volume or looseness in the pocket between the chin and the neck. Several factors can create it, often working together.
Submental fat
This is the classic cause. A layer of fat sits under the chin, and for some people it’s simply more generous there. It can be stubborn, meaning it stays put even when the rest of the body slims down.
Genetics
Where you store fat, how your jaw is shaped, and how your neck is positioned relative to your chin are all heavily inherited. Plenty of patients tell us their mother or father had the exact same profile. A recessed or smaller chin bone can also make a normal amount of fullness look more pronounced.
Weight
Weight gain adds volume everywhere, including under the chin. When excess weight is genuinely part of the picture, addressing it changes the submental area along with everything else. This is different from isolated stubborn fat on an otherwise lean person.
Skin laxity
Skin loses collagen and elastin with age. When the skin under the chin and along the jaw loosens, it sags and creates the appearance of a double chin even when there’s very little fat present. This is a texture and support problem, not a fat problem, which is why fat-focused treatments do nothing for it.
Posture and aging
Years of looking down at phones and screens, sometimes called “tech neck,” can weaken and stretch the tissue under the chin. Add the natural downward shift of facial fat and bone that comes with age, and the jawline softens. Posture won’t create a double chin on its own, but it contributes to how the area ages.

Here’s where we want to be straight with you. No single treatment fixes every kind of double chin, and some of the options you’ll read about elsewhere aren’t on Olympia’s menu. We’d rather tell you that plainly than pretend otherwise.
Addressing loose skin with RF microneedling
When skin laxity is the main issue, tightening the skin matters more than removing fat. Olympia uses Sylfirm X, a radiofrequency (RF) microneedling device that delivers energy into the deeper layers of the skin through tiny needles. That controlled energy prompts the skin to build new collagen over the following weeks and months.
The clinical research on RF microneedling in this exact region is encouraging. A 2022 prospective study in Dermatologic Surgery treated the lower face, jawline, and neck and found that physicians, blinded reviewers, and patients all rated the outcomes as highly improved, with low to no downtime.[^1] It’s important to note we use Sylfirm X specifically. We do not use Morpheus8, which is a different branded device you may see marketed elsewhere.
RF microneedling works best for mild to moderate laxity. It firms and smooths. It does not melt away a thick fat pad.
Supporting the jawline with biostimulators and filler
A soft jawline can make a double chin look worse than it is. Defining and supporting the jaw sometimes does more for the profile than treating the fullness directly. Olympia uses biostimulators like Radiesse and Sculptra, which are made to trigger your own collagen production and add structure over time. When weight is genuinely a factor
If excess weight is contributing, treating the submental area in isolation misses the bigger picture. Meaningful weight loss reduces fat throughout the body, and the face and neck are no exception.[^3] One honest caveat: rapid weight loss can sometimes leave skin looser, which is why we often pair weight management with skin-tightening down the line. What Olympia does NOT offer
This is the part we want to be direct about. Two common treatments for isolated submental fat are injectable fat-dissolving deoxycholic acid (Kybella) and CoolSculpting. Deoxycholic acid does have published evidence for reducing submental fat versus placebo, though studies also report side effects like swelling, numbness, and firmness, and every trial to date was industry-sponsored.[^4] These treatments exist and some clinics offer them. Olympia does not.
What that means for you: if a consultation shows your main issue is a significant, isolated pocket of submental fat, and skin tightening or jawline support won’t get you where you want to be, the honest answer may be a referral to a provider who offers fat-reduction procedures. We’d rather point you in the right direction than sell you something that isn’t the right fit.

Realistic Expectations
A few honest points worth setting before any treatment:
- Results build gradually. Collagen-based treatments like RF microneedling and biostimulators work over weeks and months, not overnight. Most people need a series, not a single session.
- The cause dictates the ceiling. Tightening skin can’t remove a large fat pad, and dissolving fat can’t fix loose skin. Matching the treatment to your actual cause is what produces a satisfying result.
- Genetics set the baseline. If a recessed chin or inherited jaw shape is part of the picture, treatments improve the profile but won’t rebuild bone structure.
- Maintenance is normal. Skin keeps aging and fat can return with weight changes. Good results are maintained, not frozen in place.

Can you get rid of a double chin without surgery?
Often, yes, depending on the cause. Why do I have a double chin even though I’m not overweight?
Because a double chin isn’t only about weight. Genetics, stubborn submental fat, loose skin, and a smaller or recessed chin can all create fullness on a lean person. This is common and it’s exactly why treatment should be matched to the cause rather than assumed to be a weight issue.
Does Olympia offer Kybella or CoolSculpting for a double chin?
No. Neither injectable fat-dissolving deoxycholic acid (Kybella) nor CoolSculpting is on our menu. Will losing weight fix my double chin?
If excess weight is a real contributor, weight loss usually improves the area along with the rest of the body. But it won’t help fullness caused by loose skin, genetics, or bone structure, and rapid loss can occasionally leave skin looser.
